Adding Custom Questions to an Existing Form

What's my access level?

Access Level: Publisher or Administrator

This article is intended for users with Publisher or Administrator access in Suitable. If you don’t have one of these roles, some of the steps outlined below may not be available in your account.

Ineligible Roles: Student, Contributor, Advisor

Step 1: Access Forms from the Left Side Bar in Suitable

• Click on the 'Forms' tab and locate the form for which you want to add questions.

Step 2: Select Form to Edit

• Click on the actions menu represented by three dots associated with the form.
• From the dropdown menu, choose ‘Edit Form’ to make changes to the selected form.

Step 3: Add a New Question

• Scroll to the bottom of the form. Click on the 'Add Question' button. 
• Note that each university can only utilize one set of custom questions for a NACE Ready form, which must be included at the end.

Step 4: Choose the Question Type

• Use the dropdown menu to select the type of question you want to add.
• Question Types Available:
  • Additional Instruction
  • Consent Check Box
  • Currency
  • Date and Time Selection
  • File Upload
  • Matrix Table
  • Multiple Select Check Boxes
  • Ranking
  • Single Select
  • Text Entry

Step 5: Enter Question Title and Description

• In the text boxes provided, input a title and description for your new question to ensure clarity.

Step 6: Add Answer Options

• Depending on your question type selected, type your first answer option into the designated field. To add more options, click on 'Add Option' and continue to input additional responses as needed.

Step 7: Add More Questions

• To include more questions, click 'Add Question' again and repeat the process for each new question.

Step 8: Save and Publish the Form

• Once you have finished adding questions and making necessary changes, click on ‘Save Published Form’ to save and finalize your updates.
