This article is intended for users with Publisher or Administrator access in Suitable. If you don’t have one of these roles, some of the steps outlined below may not be available in your account.

Creating a New Form
Step 1: Access the Forms Section
- Click on the 'Forms' tab in your dashboard.
Step 2: Create a New Form
- Click on ‘Create New Form’ to start the form creation process.
Step 3: Fill in Form Title and Description
- Enter a title and description for your new form in the provided textboxes to ensure clarity and context for users.
Step 4: Add Questions
- Use the dropdown menu to select the type of question you want to add.
- To include more questions, click 'Add Question' again and repeat the process for each new question.
- Question Types Available:
- Additional Instruction
- Consent Check Box
- Currency
- Date and Time Selection
- File Upload
- Matrix Table
- Multiple Select Check Boxes
- Ranking
- Single Select
- Text Entry
- Note: add answer options if necessary
OPTIONAL Step 5: Add Page Breaks
- Click on 'Add Page Break'.
Step 6: Preview Form
- Select 'Preview Form' to see a preview of your new form.
Step 7: Confirm Form Settings
- After completing the creation of your form, click on ‘Confirm Form Settings’ to finalize and save your new form.
